We have numerous 'default' index names on our server:
index.html index.shtml index.htm index.cgi
but it appears that the -I option only supports one file, so I get
duplicate URLs indexed.

Is there any other way other than using -unique to prevent duplicates?

You'll need to use -unique as -I only supports one form.
Remember to start on an empty database for -unique to work.
